Exchange and Return Form
In order for MyBioMarker to process your exchange and return for refund, kindly fill in this form and make sure to be as explicit as possible. After completion of this form one of our dedicated staff will review it and process your request. If the incorrect Item was supplied or it was defective from our-side, we will pay the courier, else you will receive an Online Payment email to pay for couriers and after payment your courier will be booked.
NB: Kindly Note that this request needs to be made within 30 days of purchase.
For Returns a Courier FEE of R120.00 will be billed and sent to you via email for payment before we book a collection with our Courier Services or you may use your own Courier at your own cost. A Refund will be processed once we have received the Item at our premises.
For Exchange a Courier FEE of R240.00 will be billed and sent to you via email for payment before we book a collection with our Courier Services (The Courier Guy) or you may use your own Courier at your own cost. Upon arrival of the item the required item/size/specification will be dispatched back to you.
